Welcome to HRSA's Clinical Pharmacy Services Planning Tool for safety net providers!
Clinical pharmacy services are a potentially effective tool for improving health outcomes for patients with certain conditions such as diabetes, dyslipidemia, asthma, those needing anticoagulation services, and others with complex medication regimens. If you are a safety net provider considering adding clinical pharmacy services to your ambulatory care setting, working through this tool will help you develop a plan for financing the services.
For the purpose of this tool, clinical pharmacy services are defined as the services that pharmacists provide beyond dispensing, aimed at improving health outcomes in the context of the patient’s regular health care team. Some of the benefits of having an onsite pharmacy that provides clinical pharmacy services (also referred to as medication therapy management services or MTM) include: providing individualized patient education, managing chronic diseases, medication reconciliation, formulary management, and other activities that monitor medication utilization and control drug costs.
